A current physical and concussion form must be on file in the athletic office before any students can start practice.

Please click on the link below to start the process or UPDATE and print the physical information to take to the doctor.  All pages must be printed out and given to the athletic office after the doctor has signed the physical form.



As a member of the Ohio High School Athletic Association (OHSAA), we are initiating a new digital standard for the pre-participation process, called e-PPE powered by PrivIT (electronic Pre-Participation).  The OHSAA e-PPE includes a more comprehensive health history questionnaire that is easy to use, legible, and accessible to help mitigate injury risks related to sports. The system facilitates the management of the PPE process and provides coaches, athletic trainers, and medical personnel the information they need to react quickly to health situations.

Next school year you will not need to enter all of the physical information again. The information will be stored and the process will be much easier, quicker to update and print.


Don't forget to complete your Concussion Form!

All parents/guardians and athletes must sign and return to the athletic  office the Concussion Information Sheet before they can participate in practices or contests.

Concussion Regulations

It is important for all involved in interscholastic athletic at Ottawa Hills Junior, Senior High School to recognize the potential for catastrophic injury and even death from concussion. It is the ultimate responsibility of the coaching staff in all sports to ensure that students are only put into practice or contest if they are physically capable of performing. However, all individuals involved in the conduct of interscholastic competition have responsibilities in this endeavor.

This information sheet is providedto assist you and your child in recognizing the signs and symptoms of a concussion. Every athlete is different and responds to a brain injury differently, so seek medical attention if you suspect your child has a concussion.

Once a concussion occurs, it is very important your athlete return to normal activities slowly.

Please consult with our Athletic Trainer or your family physician for information.
