Did You Know:
You can help to support OHPAC by designating your United Way gift to them. Here's how:
1. You may contribute to United Way individually or through your employer.
2. You can complete a paper or online form (www.unitedwaytoledo.org/give) Click on “Download Pledge form”.
3. On the pledge card, go to Step 4 (optional), which is on the back of the hard copy, Option C, and write the ENTIRE NAME “Ottawa Hills Prevention Advisory Committee” and the amount on the card
